What is Testicular Pain?

Testicular pain can affect anyone with testicles, at any age. Testicles are egg-shaped reproductive organs that are located inside the scrotum behind the penis. Most people assigned male at birth have two testicles, and pain can be felt in either or both. While pain can originate and be felt in the testes, there are instances where the pain can be coming from another part of the body, such as the groin or stomach. This is called referred pain. 

Pain can be described as acute or chronic. Acute pain means that it develops suddenly and doesn’t last very long. Chronic refers to pain that gradually increases and lasts for a significant amount of time. Due to the fact that the testes have many sensitive nerves, the pain can be severe. Most often, the pain is reported as sharp and shooting, or a constant and dull ache. 

It is not normal to experience pain in the testicles. While temporary pain can be expected after a traumatic event, any pain that lasts longer than an hour would require immediate attention as there is a need to ensure there isn’t a larger medical issue causing the pain. 

What Causes Testicular Pain?

There are many reasons why a person might experience testicular pain, the most often being trauma, which usually happens during physical or sports-related activity in younger men. This type of pain is usually temporary. 

Infections are the next most common cause of testicular pain. The most prevalent is epididymitis, which is an infection or inflammation of the epididymis (the duct through which sperm passes). This is generally caused by STDs or an enlarged prostate. Most patients who have epididymitis report a scrotum that swells and feels hot to the touch. The infection can last for weeks. Additional symptoms can include fever, vomiting, painful urination and painful intercourse. If left untreated, this condition could lead to orchitis, which can also be caused by the mumps virus.

An example of referred pain is an inguinal (groin) hernia. This happens when a part of the intestine pushes through a weak park of the abdominal muscle near the groin. Symptoms include pain and bulging in the groin and scrotum that is worse with heavy lifting. 

Less common causes of testicular pain include diabetic neuropathy, kidney stones, UTI, testicular cancer, mumps, scrotal masses and more. 

When is Testicular Pain Serious?

While all causes of testicular pain can be described as incredibly painful, there are a couple instances where the pain requires immediate medical attention. The first is testicular torsion, which happens when the spermatic cord, which provides blood flow to the testicle, rotates and becomes twisted. Severe pain, vomiting and swelling can be experienced, and immediate care is required to save the testicle. The other option is testicular rupture, usually caused by trauma to the testicles, which requires surgical intervention. 

Again, if pain is felt for longer than an hour, or the pain is unbearable and due to an acute traumatic injury, it’s important to seek the help of a medical professional right away.
